This lecture discusses how the rapid development of antibiotic resistance necessitates the advancement of novel therapeutic strategies to treat infection including synovial sepsis. Intra-articular antibiotic administration is off-label and potentially chondrotoxic, prompting re-evaluation of evidence-based dosing strategies. Orthobiologic therapies such as mesenchymal stromal cells (MSC) and platelet lysate possess antimicrobial and immunomodulatory properties that may be leveraged to improve outcomes. This lecture summarizes updated literature on novel and combinatorial therapeutic strategies to treat synovial sepsis, including the overall pros and cons of available approaches.
