The reconstruction of defects created by tumor excision can be a frustrating aspect of oncologic surgery. In general, the first surgery has the best chance of complete tumor removal. If the surgeon imposes limitations on the aggressive removal of the tumor because of skin closure concerns, the risk of recurrence will be much higher. This lecture discusses numerous reconstructive options. These options include methods to relieve tension on the suture line, skin mobilization techniques, various skin flaps, and the use of full and split-thickness skin grafting. This lecture provides attendees intra-operative flexibility and the latitude to be more aggressive during primary resection.
